At Telecare’s Santa Clara Assisted Outpatient Treatment (AOT) program, our goal is to work closely with existing community resources to assist individuals with transitioning back into their community. We know that the journey to recovery can be a tough one, and we believe that no one should have to go through it alone.

Program Type: Community-Based

To Make Referrals: Call Behavioral Health Call Center at 1-800-704-0900 and select option 7 to speak with an AOT team member. You can also visit the Santa Clara County Behavioral Health Services website (bhsd.sccgov.org/get-help/assisted-outpatient-treatment-aot) and fill out an AOT Referral Form and email it to AOT@hhs.sccgov.org.

Number of Members Served: 100 members

Who We Serve:

  • Adults ages 18 or older and has a severe mental illness.

  • Clinically determined that, in view of the person’s treatment history and current behavior, at least one of the following

    is true:

    • The person is unlikely to survive safely in the community without supervision and the person’s condition is substantially deteriorating.

    • The person is in need of AOT in order to prevent relapse or deterioration that would be likely to result in grave disability or serious harm to the person or to others, as defined in Section 5150.

  • Has a historical lack of compliance with voluntary treatment and at least one of the following is true:

    • Within the last 36 months, the person has been hospitalized at least 2 times due to mental illness, including the mental health unit of a state or local correctional facility.

    • Within the last 48 months, had one or more acts of serious and violent behaviors or threats or attempts to cause serious physical harm to themselves or others.

  • Offered an opportunity to participate in a treatment plan but did not engage.

  • Participation in AOT is the least restrictive placement necessary to ensure recovery and stability.

  • It’s likely that the individual would benefit from AOT.

Hours of Operation:

  • Monday - Friday: 8:30 a.m. to 5:00 p.m.

  • On-call staff availability 24 hours a day, 7 days a week

About the Program: Based on a recovery-centered model, AOT of Santa Clara County is an intensive community support service and an Assertive Community Treatment (ACT) for individuals with severe mental illness (SMI), many of whom would otherwise require extended care in institutional settings. AOT serves individuals who are high utilizers of mental health services and who are considered to be at great risk for psychiatric hospitalization. Our multidisciplinary team includes a psychiatrist, a nurse, masters-level clinical staff, and case managers who are all here to help individuals in our program on their path.

AOT staff will walk with individuals on their journey in healing and provide a full range of services, including medical and psychiatric services, case management services, advocacy and linkage, referral to safe and affordable housing, substance use interventions and counseling, assistance with entitlements, support and education with family and significant others, connection with community resources, and self-help groups.
